CSW Program

The Community Service Worker (CSW) program is dedicated to reaching out to the most vulnerable members of our community through street outreach initiatives. Utilizing the unique approach of credible messengers—individuals who have a profound understanding of the challenges faced by those we serve—our program aims to provide comprehensive support and bridge critical gaps in care.

About The Program

The PM Foundation was awarded the BIPOC (Black & Indigenous People of Color) PA State Grant to Reduce Opioid Overdose Rates. The grant provided the funding to create the Community Service Worker Program (CSW), the first of its kind in Pennsylvania.

CSWs provide 1:1 support to engage clients in the community and in their “natural” setting, which can vary significantly depending upon their situation. These efforts help connect clients to essential treatment, services, and support, eliminating barriers and reducing the high-risk behaviors often associated with mental illness and drug addiction. With just over a year since the program's inception, we've already made a significant impact.
